Clinical systems · AI medical scribe (SMB)
Twofold Health
Twofold Health is an AI medical scribe that turns recorded visits into clinical notes with CPT and ICD-10 code suggestions, for a flat monthly fee.
Strong fit
- Therapists and behavioral health clinicians who write many notes
- Solo and small-practice clinicians who want a flat monthly price
- Clinicians who want CPT and ICD-10 code suggestions with each note
Weak fit
- Health systems that need deep EHR integration
- Buyers who want a long track record and outside studies
- Clinicians who want a permanent free plan
Bottom line
Twofold is a good value for solo and small-practice clinicians, especially therapists, who want unlimited AI notes and code suggestions for a flat $49 a month. It suits self-serve buyers who can start with a free trial. Health systems that need deep EHR integration or published outcome studies should look at larger scribes.

Twofold is an AI medical scribe. A clinician records an in-person or telehealth visit, and Twofold writes the note, such as a SOAP or progress note, and suggests CPT and ICD-10 billing codes. It separates several voices in couples and family sessions and offers more than 70 templates.
Much of its early growth came from therapists and other behavioral health clinicians, and it now serves primary care, pediatrics and hospital medicine too. Twofold says more than 20,000 clinicians and 3,000 practices use it.
Twofold does not store audio, and every plan includes a signed business associate agreement, the HIPAA contract that covers patient data. Group accounts give practice leaders a dashboard of each clinician's note progress. Twofold lists SimplePractice, TherapyNotes, athenahealth and Epic among the EHRs it works with.
Heidi Health and Tali AI fit when a clinician wants a free plan to start. Freed fits small ambulatory clinics that want a scribe built for them, and Nabla fits groups that want notes written back into the EHR.
