Prior authorization & UM · Health plan clinical review AI
Anterior
Anterior makes clinical AI for health plans that reviews prior authorization requests and other clinical work against plan guidelines, approving clear cases in minutes and preparing the rest for nurse review.
Strong fit
- Health plans whose nurses and medical directors review a high volume of prior authorization requests
- Plans that want one clinical AI for utilization management, appeals, payment integrity, and risk adjustment
- Plans on HealthEdge GuidingCare that want a ready integration
Weak fit
- Hospitals and practices that submit authorization requests to payers
- Small plans without a clinical review team to oversee the AI
- Buyers who need a published price list
Bottom line
Anterior makes clinical AI for health plans that reads medical records against coverage guidelines, approves clear-cut prior authorization requests in minutes, and sends the rest to nurses with the evidence laid out. The company places its own clinicians inside each plan's review team during rollout. It fits health plans that want faster approvals and more output from the clinical reviewers they have.

Abdel Mahmoud, a doctor who left medicine after growing frustrated with paperwork, founded Anterior with Zahid Mahmood, a software engineer who had built payment systems for large insurers. The two became friends at University College London. The company first operated as Co:Helm and went through Sequoia's Arc program.
Its system, Florence, reads faxes and medical records and checks them against each plan's guidelines. It also turns policy documents into decision logic.
Clear cases are approved automatically. The rest go to a nurse or medical director with the relevant evidence pulled together.
Under its Forward Deployed Clinician model, Anterior embeds clinicians and engineers with each customer. They tune the system to the plan's own policies and workflows.
Sequoia led its seed round. A $20 million Series A followed in June 2024, and a $40 million round in February 2026 with NEA, FPV Ventures, and Kinnevik brought total funding to $64 million.
Anterior works with plans covering 50 million lives, including Geisinger Health Plan. It connects to HealthEdge GuidingCare and has a partnership with Stellarus.
KLAS Research validated its clinical accuracy at 99.24%. One enterprise customer cut clinical review cycles by about 75% after rolling it out across hundreds of nurses, with staff satisfaction above 90%.
Some cancer care requests are approved in roughly 155 seconds. Average deployment takes five days.
Xsolis fits a plan or hospital that wants AI help with inpatient status decisions and working with the other side. Banjo Health or Itiliti Health fit a plan that wants software to automate authorization rules and intake.
Agadia fits when much of the volume is pharmacy benefit requests. Hindsait fits when reviewers mainly need software that reads unstructured charts and matches them to criteria.
