Revenue cycle · Payer payment integrity AI
Codoxo
Codoxo is a payment integrity software company in Duluth, Georgia, whose AI platform helps health plans, government agencies, and pharmacy benefit managers find billing errors, fraud, waste, and abuse in claims, and warn providers about coding problems before they bill.
Strong fit
- Health plans that want fraud detection, claim audits, and case management in one platform
- Payment integrity and special investigations teams that need to review more claims with the same staff
- Plans that want to warn providers about coding problems before claims arrive
Weak fit
- Hospitals and physician practices looking for billing software of their own
- Small plans without investigators or auditors to follow up on alerts
- Buyers who want published prices before a sales call
Bottom line
Codoxo is an AI payment integrity platform for health plans, government agencies, and pharmacy benefit managers. It suits payers that want fraud detection, claim audits, and provider education in one system and have their own teams to act on what it finds. Providers looking for billing tools of their own should look elsewhere.
Score breakdown
Weights: Outcomes 35% · Product 30% · Implementation 20% · Pricing clarity 15%.
Codoxo is bought by health plans, state and federal agencies, and pharmacy benefit managers. Their payment integrity and special investigations unit (SIU) teams use it to spot suspicious billing patterns, audit claims, manage fraud cases, and send providers education about coding mistakes. It analyzes professional, facility, and pharmacy claims, and it is built for plans that run these programs in house with their own investigators and auditors.
Codoxo sells enterprise contracts and does not publish prices. It is a smaller, venture-backed alternative to large payment integrity firms such as Cotiviti, with an estimated $30 million in annual revenue. Among companies we review, Certilytics builds predictive analytics for health plans, Availity runs payer connectivity and prior authorization tools, and Sift Healthcare works the other side of the claim, predicting denials for hospitals.
Musheer Ahmed founded the company in 2017 as FraudScope, using patented AI he developed during his Ph.D. at the Georgia Institute of Technology, and renamed it Codoxo in December 2020, Hypepotamus reported. Mustaque Ahamad, a Georgia Tech computer science professor who also co-founded Pindrop Security, is co-founder and chief scientist.
Its products run on one system that Codoxo calls the Unified Cost Containment Platform. Fraud Scope looks for new fraud schemes and gathers evidence for investigators, Payment Scope flags claim accuracy problems before payment, Audit Scope runs audit workflows, and Clinical ClaimPilot uses generative AI to speed up medical record reviews. Provider Scope sends providers automated education about coding problems before they bill, an approach Codoxo calls Point Zero payment integrity because it acts before a claim exists.
In its February 2026 Series C announcement, Codoxo reported 100% customer retention and 125% net revenue retention in 2025 and said Point Zero customers save $12 to $16 per member per year. The $35 million round was led by CVS Health Ventures, with Echo Health Ventures joining. The same release announced Deepfake Detection, which looks for synthetic medical documents and altered images, and a Policy-to-Code tool that turns payer policy language into code. Codoxo holds HITRUST r2 certification and says it is working toward FedRAMP readiness, which matters for federal agency contracts.
By October 2026 Codoxo said its platform supported payment integrity for more than 167 million members, and TIME included it in its list of the World's Top HealthTech Companies 2026.
What customers say
What people like
"Our SIU team gets alerts on new schemes with the evidence already gathered, so investigators start on the case instead of on a spreadsheet."
"Being able to warn a practice about a coding pattern before the claims come in means fewer clawbacks and fewer angry calls from providers."
What people don't like
"There is no price on the website, so we sat through demos before we had any sense of what it would cost."
"It is built for plans with their own payment integrity staff, so a small plan still needs investigators to follow up on every alert."
