AffirmedRx

AffirmedRx is a pharmacy benefit manager in Louisville, Kentucky, that runs prescription drug benefits for self-funded employers and other plan sponsors for a flat fee per claim, passes all drug maker rebates to its clients, and is organized as a public benefit corporation.
Strong fit
- Self-funded employers that want full claims data and visibility into their drug spending
- Plan sponsors leaving a large PBM over rebate retention or spread pricing
- Plans that want a team following up on rejected claims before members reach the pharmacy
Weak fit
- Members who rely on brand drugs left off its formulary
- Plans that want a PBM with decades of operating history
Bottom line
AffirmedRx is a transparent pharmacy benefit manager that earns a flat fee per claim, passes every rebate back to the plan, and gives clients their full claims data. It is worth a serious look for self-funded employers that want to see and control what they pay for drugs. Members can still pay more when a plan changes its coverage rules, as some Purdue University employees reported after the university switched to AffirmedRx.

AffirmedRx is bought by self-funded employers and other plan sponsors that pay for their members' prescriptions directly. As their pharmacy benefit manager (PBM), it builds the formulary, processes pharmacy claims, contracts with pharmacies, and handles rebates from drug makers. Purdue University moved its retail pharmacy benefit to AffirmedRx from CVS Caremark, WTHR reported, and in 2022 the company partnered with Mark Cuban Cost Plus Drugs to offer its lower-cost drugs to employers, according to a joint announcement.
AffirmedRx competes with the large PBMs and with other transparent PBMs. It is paid a flat dollar amount per paid claim, so it earns the same whether a drug costs $50 or $5,000, CEO Greg Baker told Managed Healthcare Executive. It passes 100% of rebates to clients and gives them all their data. Among companies we review, Scripta Insights and Arine help plans lower drug costs alongside a PBM, and Agadia sells prior authorization software to health plans and PBMs.
The company started as EmsanaRx, launched by the Purchaser Business Group on Health, and changed its name to AffirmedRx in May 2023. Baker, a pharmacist who spent 13 years at Walgreens and ran employer-site pharmacies at Premise Health, founded it in October 2021, Inc. reported. Revenue reached $383.6 million in 2025, a three-year growth rate of 36,730%, and AffirmedRx ranked No. 5 on the 2026 Inc. 5000. As a public benefit corporation, it caps its profit at 3%, according to Inc.
Its patient care advocates look at every rejected claim and contact the doctor, pharmacist, and patient, so the member does not learn about the rejection at the pharmacy counter, Baker told Managed Healthcare Executive. The company runs on a cloud-based claims system and built its own formulary through its pharmacy and therapeutics committee.
Public member feedback is limited. The AffirmedRx Pulse app has a 4.6 rating on the App Store from five ratings. In September 2025, WTHR reported that some Purdue employees were paying more after more than 700 medications were moved to a non-preventive list under the new plan; minutes from Purdue's board of trustees put the health plan's savings at $1.67 million, and Purdue said it covered 95% of the overall increase in health care costs for 2025.
