Groups is used by adults with opioid use disorder, many of them on Medicaid, who want medication and a recovery community in one program. After a virtual intake with a counselor and a physician, a member gets a Suboxone prescription filled at a local pharmacy and joins a weekly group led by a licensed counselor, online or in person. In some states Groups also prescribes long-acting injectable medication.

Groups is paid for mainly through insurance. It accepts Medicaid, Medicare, and most commercial plans, and offers discounted self-pay options and state grant programs for uninsured patients in eligible states, but it posts no cash price. QuickMD charges $99 per visit in cash, and Bicycle Health lists $249 a month for self-pay patients.

The closest competitors are Bicycle Health, Ophelia, Boulder Care, Workit Health, and QuickMD.

Dr. Jeff De Flavio co-founded Groups in 2014, when he opened an office to treat a patient with heroin addiction, according to its About page. Today it has more than 120 locations and serves 13 states. Cooper Zelnick, who joined in 2018 and served as president and chief strategy officer, became chief executive in September 2025. He told Behavioral Health Business that Groups is the largest value-based provider of its type in the country.

Treatment has firm rules. Members complete weekly urine screens through the Groups app, which Groups says many states require for Suboxone treatment, and attend a weekly group. Zelnick has said Groups is investing in free and discounted programs for uninsured patients and in exchange plan contracts as Medicaid enrollment tightens.

What patients say

"Having a group every week with the same people is what kept me showing up."

"My Medicaid covered it, so the hardest part was making the first phone call."

"Weekly urine screens through the app felt like a lot once I was stable."

"There was no price on the site, so I had to call to find out what self-pay would cost."
