Revenue cycle · Clinical compliance and chart audit AI
Brellium

Brellium is a New York City clinical compliance company whose AI audits every patient note for documentation, coding, and payor-policy issues before claims go out.
Strong fit
- Behavioral health and multi-specialty groups that sample only a fraction of charts today
- Providers under rising payor and CMS audit pressure
- Organizations that want under-coding and over-coding caught before submission
Weak fit
- Solo practices that bill a low volume of simple visits
- Teams that cannot start without a public price list
- Teams that want a full outsourced coding service instead of pre-bill software
Bottom line
Brellium reads every clinical note against payor, regulatory, and quality rules, and it says it has audited more than 22 million visits and protected nearly $2 billion in revenue. In October 2026 it raised $23 million and launched CortexLM. Customers such as Headspace report moving from sampling a few percent of notes to reviewing all of them, and published pricing is not available.
Score breakdown
Weights: Outcomes 35% · Product 30% · Implementation 20% · Pricing clarity 15%.
Brellium's buyers are provider groups and national clinical organizations. Compliance, quality, and revenue integrity teams use it to review every chart before billing, flag documentation and coding gaps, and keep an audit-ready record. The company says it supports more than 215,000 providers and 200 provider groups across all 50 states, including Gentiva, Maxim Healthcare, Rula, and Headspace, according to an October 2026 Business Wire release.
Pricing is by quote. Among companies we review, SmarterDx and CodaMetrix use AI on clinical documentation and coding, Fathom automates medical coding, and Silna Health focuses on prior authorization readiness for specialty providers. Brellium's pitch is full-population pre-bill compliance instead of coding alone.
Zach Rosen co-founded Brellium in New York City in 2021 and is CEO, according to its about page. Co-founders include Henry Kasa and Max Katzman. In April 2025 it raised $16.7 million, and in October 2026 it raised another $23 million led by Left Lane Capital and First Round Capital to expand into cardiology, pain management, and primary care and go deeper into the revenue cycle.
The October 2026 release said Brellium had audited 22 million patient visits and protected nearly $2 billion in provider revenue from recoupments. Headspace scaled chart review from 2% of therapy notes to 100% and reports 99.2% provider compliance with documentation standards. Brellium has an estimated $12 million in annual revenue.
Independent clinician review sites have little coverage of Brellium so far. Published customer comments come mainly from the company's own site and press materials, where medical directors describe reviewing every visit and cutting chart-review time.
