Population health & analytics · Risk adjustment and quality platform
Reveleer

Reveleer is a value-based care software company in Glendale, California, whose platform helps Medicare Advantage health plans and risk-bearing provider groups retrieve medical records, code risk adjustment diagnoses, close quality gaps, and show suspected conditions to doctors inside the EHR.
Strong fit
- Medicare Advantage plans running risk adjustment and quality programs at scale
- Risk-bearing provider groups that want suspected conditions and care gaps shown to doctors in the EHR
- Plans preparing for CMS risk adjustment data validation audits
Weak fit
- Small practices with few value-based contracts
- Buyers who need one unified member view across every module
- Teams without staff to own a long setup
Bottom line
Reveleer is a value-based care platform for health plans and risk-bearing providers that combines chart retrieval, AI-assisted coding, quality programs, and alerts at the point of care. It is worth a look for Medicare Advantage plans that want one vendor across risk adjustment and quality, with AI suggestions that link to the page of the record they came from. Customers describe it as a strong partner, and setup is where they report the most friction.

Reveleer is used by Medicare Advantage health plans, accountable care organizations, and provider groups that are paid according to how sick their members are and how well they score on quality measures. Plans use it to retrieve medical records, code diagnoses for risk adjustment, close care gaps for quality programs, and prepare for audits. In a May 2026 THL podcast, CEO Jay Ackerman described the core job: a plan is paid a monthly amount per member based on that member's health burden, and Reveleer helps the plan document that burden from clinical records so the government pays the right amount.
Reveleer is an enterprise vendor priced for health plans. Ackerman told Founderpath that the company moved to a per member per month model and that average contract values were approaching $900,000 a year. Among companies we review, Cozeva and Navina cover provider-side quality and risk work, Persivia sells a broader population health platform, and Moxe Health moves clinical records between hospitals and health plans.
Reveleer started in 2009 in medical record retrieval. When Ackerman joined in 2016, the company was called Health Data Vision. Revenue doubled from $25 million in 2022 to $51 million in 2023, Ackerman told Founderpath, as the customer count grew from about 30 in 2019 to about 80, and Founderpath now lists company revenue of $95 million. The company has made four acquisitions and has about 450 employees, Ackerman said on the THL podcast.
Its EVE AI links each coding suggestion to the exact page in the medical record where the evidence appears, so coders and clinicians can check it before they accept it, according to an October 2026 announcement. Reveleer says it has processed more than 2 billion pages of records and that its programs serve 12 million covered lives. On the THL podcast, Ackerman said its hybrid AI pairs Google's Gemini with a rules engine of more than 1,500 clinical rules, and that customers expect precision of 85% or better.
Comments on KLAS praise Reveleer as a partner that keeps up with regulatory mandates. The complaints are about implementation and training, separate modules that do not share one member view, and add-on charges.
