Patient engagement & telehealth · Remote patient monitoring
Vivo Care
Vivo Care runs remote patient monitoring and chronic care management programs for practices, with cellular devices, software and a US nurse team.
Strong fit
- Independent practices that want RPM and CCM run by outside nurses
- Clinics with older patients who do better with cellular devices
- Practices switching from an RPM vendor that shut down
Weak fit
- Health systems that want to staff programs in-house only
- Buyers who want published outcomes data
- Buyers who need a public price list
Bottom line
Vivo Care fits independent practices that want remote monitoring and chronic care management run largely by its US nurse team. It is newer under this name and publishes little outcomes data. Ask for references and contract terms before you sign.

Vivo Care is the former Optimize Health, a Seattle company founded in 2015 that relaunched under the new name in August 2025. Ryan Clark, who joined in 2021, is chief executive.
Vivo Care runs remote patient monitoring and chronic care management for practices. It ships cellular devices to patients, checks insurance eligibility, enrolls patients in clinic or by phone, and tracks time and billing codes in its software.
Its nurses are licensed in the US and make up more than 80% of the company. They watch readings and call patients, and Vivo Care says they resolve about 95% of alerts without the practice.
Vivo Care says it has supported more than 100,000 patients and serves practices in all 50 states. A Colorado internal medicine practice moved to it in 30 days after its earlier vendor shut down.
Foundry and Escalate Capital Partners led an $18 million Series B in October 2023, when the company was still Optimize Health.
Health Recovery Solutions fits when a health system or home health agency wants its own clinicians working from a patient app and dashboard. HealthSnap and CoachCare fit when a practice wants a similar nurse-backed model to compare against.
