Samson Magid co-founded HealthSnap in 2015 with Chase Preston and Wesley Smith, out of University of Miami research on improving health through lifestyle and technology. Magid has said the idea came from his grandfather, whose heart failure and type 2 diabetes went largely unmonitored between appointments. HealthSnap launched remote patient monitoring in 2019 and later added chronic care management. Its software takes readings from cellular blood pressure cuffs, scales, and glucose meters, and its own clinical team can enroll patients and make the monthly calls.

HealthSnap raised a $25 million Series B led by Sands Capital in February 2024, which brought total funding to $48.5 million. In August 2026 it added a $25 million debt facility from Eastward Capital Partners and said it works with more than 200 health systems and physician organizations.

Compare Health Recovery Solutions when a health system needs post-discharge and hospital-at-home monitoring, Prevounce Health when a smaller practice wants help with billing rules, and Smart Meter when the team only needs cellular devices for its own software.
